3 Things about, common cause of increased urination in elderly men

Benign Enlargement of Prostate / Benign Prostatic hyperplasia (Non –Cancerous)

In this Post you will get answers to the following questions.

Why does the Prostate enlarge?

What are its symptoms?

What is the treatment for it?

Prostate is a gland below the urinary bladder i.e. urine bag in males.

It produces a fluid which is a part of the semen.

When this gland enlarges in size, it squeezes the urethra, i.e. a tube which carries urine out of the bladder. It is seen in males over 50 years of age.

1.Why does Prostate enlarge?

Prostate is governed by the male and female hormones in which the male hormones have an upper hand .

There are various theories about its enlargement. One theory says that, as age advances the male hormones diminishes but the female hormone oestrogen is not decreased equally.

Thus prostate enlargement occurs due to a disturbed ratio of the circulating male hormones i.e. androgen and female hormone, i.e. oestrogen.

2.What are the symptoms?

  1. Frequency of urination is the first symptom. Initially there is increased frequency at night. A person may have to wake up 2 times or more than that for urinating. It is usually around 2 or 3am.

Then it will progress to daytime also. So both during the day and night the patient will have to go to urinate frequently

  2. Urgency- Later it starts becoming urgent. He has to rush to the toilet. Furthermore there is dribbling of urine. It might lead to cystitis i.e. inflammation of the urinary bladder i.e. urine bag.
  3. Difficulty in urinating. He has to wait for the urine to expel, there is no use of straining.
  4. The stream is weak, sometimes dribbling to the end.

  5. Pain and fullness in the lower parts due to cystitis or acute retention of urine (urine has been collected in the bag and is not let out)
  6. Blood in urine. Occasionally there might be a drop of blood at beginning or end of urination.

The patient might get recurrent urine infections as the urine stays in the bladder ( urine bag) and is not able to get expelled out.

Routine Tests

A sonography (USG) of  pelvis is performed, before and after urination. Findings – Prostate volume >30ml, Post void urine >50ml

A blood test PSA ( to rule out prostate cancer)

3.What is the treatment ?

1.      Medicines- Tablets to relax the neck of the bladder muscles , so that urine flows smoothly. Others to shrink the prostate. A few combination of medicines can be used.

2.      Surgery to remove the Prostate gland
